Work Permits
To obtain a work permit submit the following documents to either Ms. Moffat (Green Student Services Office) at tmoffat@d125.org, Ms. Whalen (East Student Service Office) at bwhalen@d125.org, or Ms. Pankiewicz (Gold Student Services Office) at jpankiewicz@d125.org.
- Completed Work Permit Application form. Your employer may submit a letter of intent instead of completing the “Employer Information” section if the letter includes all the information requested on the form, including the employer's signature.
- Copy of your Birth Certificate.
- Please have the school nurse complete the Statement of Physical Fitness or provide a copy of your most recent Certificate of Physical Fitness.
We will electronically file your permit with the State of Illinois and send a copy to the email provided on the application form.
*Please note that at this time, we can only issue work permits for our current students.
For any other in-district residents with younger children requesting a permit, please contact your student's middle school or the Lake County Regional Office of Education at 847-665-0603 or 847-665-0609 for assistance.
