Proof of Residency
According to Illinois School Code, only students who reside within the attendance boundaries of District #125 may enroll at Stevenson High School. Residency within the district is verified at the time of a student’s enrollment as well as during each spring for the subsequent school year. The District contracts with a third-party vendor to verify student address information by comparing parent/guardian names and addresses with electronic databases. Additionally, the district may also require additional proofs of residency, interviews of the person(s) enrolling the student, and/or home visits to verify a student’s residency status.
NOTE: The process of establishing residency for newly enrolling families is different than the annual verification of residency for currently enrolled families which is required each summer. For more information about annual residency verification, please click here.

- Current and Valid Lease Agreement (including name and phone number of landlord)
- Monthly Mortgage Statement
- Real Estate Tax Bill
- Closing Document (if moved within the last 30 days)
- Bill of Sale (if moved within the last 30 days)
- Warranty Deed (if moved within the last 30 days)

Utility Bill Statement - Entire first page of the bill including the coupon portion. Utilities accepted include gas, electric, water, trash collection or cable.

Copy of Birth Certificate (with a certified translation, if not in English)

Health Records or Immunizations Record
- Current Academic Records (unofficial transcript) and Standardized Test Scores (if available)
- Translated, Official/Certified transcript and a letter of withdrawal from prior school (if coming from another country)
