Student Accident Insurance Program

Stevenson High School District 125 is providing and paying for Student Accident Insurance for the 2012-13 school year. This coverage is in force for all participants in school sponsored and supervised student activities, including interscholastic athletics and sponsored group travel, regardless of the location of these activities. Benefits are determined on the basis of reasonable and necessary for our geographical location and are payable on a 100% basis. Payment of all medical/dental expenses incurred from accidents is made in excess of any family or employer group coverage or plan benefits. This is a program of supplemental coverage designed to pick up eligible balances left by the family or employer group insurance plan and, if no other coverage or plan is available, to pay the medical/dental expenses incurred up to the policy limit of $25,000.

If the insured’s primary coverage is an HMO (Health Maintenance Organization) or PPO (Preferred Provider Organization) this plan will provide benefits in excess of coverage provided by the insured’s HMO or PPO.

The student Accident Insurance Program provides a $10,000 accidental death benefit and an accidental dismemberment benefit for accidental loss which occurs as a result of a covered accident. Finally, the District Student Insurance Program includes catastrophic injury coverage which provides a $5 million medical maximum for higher risk activity and covers all interscholastic sports, poms, band, cheerleading, intramural sports, gym classes and non-sport extracurricular activities.
In addition, Stevenson High School, in conjunction with Gerber Insurance, provides the opportunity for parents to purchase student/accident insurance for non-school related accidents, as well as dental accident coverage. Enrollment for these plans will be done online. Interested families should visit the Gerber website http://www.k12specialmarkets.com where the full details of the program are available. (When signing up, select Collective Liability Insurance Cooperative as the plan option. Stevenson is part of the cooperative.) Enrollment is done online through the same website using a credit card. Parents also have the option of printing out an enrollment form and mailing it in with a check or money order.

Claim Processing
The claim procedure is prompt and efficient. Generally, for those students covered by family coverage through a parent’s insurance plan, the Stevenson High School Student Accident Program becomes secondary coverage. For those without coverage, the Stevenson High School program becomes primary. In the event of injury, those students with existing coverage would need to file first with their primary carrier then secondly with the Stevenson High School program.  No claim will be processed without either (a) an explanation of benefits (EOB) from the primary carrier; or (b) an indication of no other source of insurance coverage.
 
The nurses’ office and the athletic office have been supplied with claim forms. In addition, claim forms are available online at http://www.k12specialmarkets.com. When there is a school-related injury, the parent or guardian will be responsible for filling out a portion of the claim form and Stevenson High School will be responsible for verifying the student’s name and the circumstance of the accident. The claim will then be submitted to Zevitz-Redfield and Associates. The parent or guardian will then submit any related medical bills along with their insurance company’s explanation of benefits to indicate the portion paid by the family or employer group coverage.
