How to Register for Athletics

Instructions for Athletics Registration

1. Download the Athletic Participation Form from the Stevenson High School Athletics website, http://www.d125.org/athletics.

2012-13 Athletic Participation Form

2. Complete The Athletic Participation form and the Past Medical History form.

3. Attach a COPY OF A CURRENT PHYSICAL.  If you are an incoming freshmen, please provide the Athletic Office and the Nurse's Office with separate copies.

4. All materials should be turned in to the Athletic Office, Room 2000, prior to the sport's first day of tryouts or practice. Forms can also be faxed (847-634-0648) or mailed.
